By filling out this questionnaire, you, the Questionnaire Participant, are agreeing to allow Kristen Renee Foundation (KRF) to share your information with partner organizations and government officials. One of our team members, or representatives, may contact you in order to gather more information or to get clarification. KRF will not sell your data or information. This data will be used for the purposes of research and statistical analysis to help the community of Frederick, Maryland. This survey is available to residents of Frederick, Maryland and former residents of Frederick, Maryland and/or those who worked in Frederick or may have spent considerable time there.

This Questionnaire is provided with no guarantees, warranties or conditions. The Questionnaire is not an obligation, explicit or implied, for good, services, monetary compensation, or payment. This Questionnaire is made available exclusively for the distribution of the Kristen Renee Foundation (KRF). If you are not acting on behalf of KRF, you may not share or distribute this Questionnaire. Kristen Renee Foundation will not be liable for any damages including, without limitation, any direct, indirect or consequential damages, any damages whatsoever arising from use of inability to use the Questionnaire, any loss of data, or any loss of profits, whether in action of contract, negligence or other tortious action, arising out of or in connection with the use of this Questionnaire.

Instructions for filling out the Kristen Renee Foundation’s “Fighting For Frederick” Questionnaire –

You can simply complete the Questionnaire online and submit it electronically. You will be notified by email when your Questionnaire is received and reviewed by a staff volunteer.

The Kristen Renee Foundation discovered that Fort Detrick sprayed Agent Orange decades ago over Area B that was surrounded by farms and residences and is now an impacted community. The Foundation met with Fort Detrick officials and their attorneys to confirm this, gaining the admission that in fact, this was done by both flyovers and spraying from the back of trucks. The Kristen Renee Foundation has obtained evidence of additional hazardous waste buried not only in Area B, but also on the Main Post Area A, and a malfunctioning treatment system for ultra-hazardous waste.

By providing information through our Questionnaire, you will be helping the Kristen Renee Foundation in the efforts to better understand how the Frederick residents have been possibly affected by the contamination of Agent Orange.
